3. Do you have cold like symptoms in the last 14 days?
4. Do you have fever like symptoms in the last 14 days?
5. Have you been in contact with anyone with the flu like symptoms?
6. Have you been in contact with anyone outside of Canada within the last 14 days?
If you've answered "Yes" to any of the above questions, please reschedule the appointment to 14 days later.
